FDA/CFSAN - Cosmetics--Hair, From Personal Statement to Personal Problem U. S. Food and Drug Administration FDA Consumer December 1991 HAIR! From Personal Statement to Personal Problem by Devera Pine From the shaved heads of medieval monks, to the long-haired hippies of the '60s, to the spiked hairdos of today's punkrockers, hair has always made a personal statement. "It's one of the leading ways people can establish their individuality and express their style," says Jerome Shupack,M.D., professor of clinical dermatology at New York University Medical Center in New York City. "Hair has had sociologicalimportance throughout the ages." Because of its importance, anything that happens to our hair that we can't control--falling out or turning gray, forinstance--can be the source of much anxiety. In the United States, some 35 million men are losing or have lost their hair from male-pattern baldness, according to theAmerican Hair Loss Council. Approximately 20 million women have experienced a similar loss of hair (from female-patternhair loss), and an estimated 2.5 million Americans have lost their hair due to other causes. THE BASICS Hair is produced by hair follicles--indentations of the epidermis (outer skin layer) that contain the hair root, themuscle attached to it, and sebaceous, or oil, glands. Hair is made up of dead cells filled with proteins, most of whichare known as keratins. The cells are woven together like a rope to form the hair fiber. The hair fiber, in turn, hasthree layers: the outer cuticle with its fish-scale-like structure; the cortex, which contains the bulk of the fiber;and the center, or medulla. Hair color is determined by melanocytes, cells that produce pigment. When these cells stopproducing pigment, hair turns gray. Although it seems as if the hair on your head is always growing, hair actually has active and rest phases. The growthphase, known as anagen, lasts for two to six years. At any given time, about 90 percent of scalp hair is in the growthstage. The remainder is in the rest phase, known as telogen; this lasts from two to three months. Once the rest phase is over, the hair strand falls out and a new one begins to grow. As a result, it's considered normalto lose from 20 to 100 hairs a day, says Diana Bihova, M.D., a dermatologist in private practice in New York City. Onlya change in your regular pattern of loss is considered abnormal--but many things, including genetic factors, diet, stress, and medications, can change that pattern. BALDNESS: MANIFEST DESTINY? The most common cause of hair loss in both men and women is rooted in a genetic predisposition. Called androgenicalopecia, it is known as male-pattern baldness in men and female-pattern hair loss in women (alopecia is the scientificterm for baldness). According to the American Hair Loss Council, genetics accounts for 95 percent of all cases of hairloss in this country. Baldness results from a combination of genetic factors and levels of testosterone (a hormone produced by the adrenalgland in both sexes and also by the testes in men). If hormone levels are right, then the hair follicles will expresstheir genetic destiny by growing for shorter periods and producing finer hairs. In men, who have higher levels oftestosterone than women, this eventually results in a bald scalp at the crown of the head and a horseshoe-shaped fringeof hair remaining on the sides. In women, the hair thins all over the scalp; the hairline does not recede. This typeof hair loss doesn't usually show up in women until menopause; until then, estrogen tends to counteract the effects oftestosterone. ONE APPROVED DRUG (See updated information below) The only drug approved by the Food and Drug Administration to treat pattern baldness or hair loss is minoxidil topicalsolution (Rogaine), which is rubbed into the scalp. Originally approved for hereditary male-pattern baldness in 1988, itwas also approved for treating female-pattern hair loss in August 1991. However, it should not be used by pregnant ornursing women. In his dermatological practice, Arthur P. Bertolino, M.D., Ph.D., director of the hair consultation unit at New YorkUniversity, says that this lotion helps hair grow in 10 to 14 percent of the people who try it. He estimates thatapproximately 90 percent of the time, Rogaine at least slows down hair loss. (Minoxidil is also available in tablet formto treat severe high blood pressure. Oral minoxidil has a potential for serious side effects and is not approved to treatbaldness.) No one is certain yet just how topical minoxidil works to promote hair growth. "One theory is that it dilates the bloodvessels, so it may stimulate nourishment of follicles," says Bihova. Alternatively, Rogaine may convert tiny hairfollicles that produce peach fuzz into large hair follicles that produce normal-size hairs. Again, no one knows for sure. What is certain is that, at least in men, Rogaine works better on patients who fit a certain profile: they've generallybeen bald for less than ten years, have bald spots on top of the head that are less than four inches in diameter, andthey still have fine hairs in their balding areas. "The process begins very early," says Bihova. "I see 19-, 20-year-oldmales who have it." The most common side effects with this medication are itching and skin irritation. Also, according to Bertolino, once youstop using it, any hair that grew as a result will fall out. Finally, the drug is expensive: in 1990, it cost about $600a year to use it twice a day. TRANSPLANTS Baldness can also be treated with hair transplants, in which plugs of "donor" follicles from the patient's scalp areused to fill the hairline. Although hair transplants work well in both men and women, the treatment tends to have a moredramatic effect on appearance in men with bald spots than it does on women with thinning hair. "The less hair you have, the more drama in the change," says Robert Auerbach, M.D., associate professor of clinicaldermatology at New York University School of Medicine. However, the American Hair Loss Council warns against attemptingto replace lost hair with hair pieces sutured to the scalp. FDA has not approved any products specifically intended forthis purpose, however, this does not preclude a physician from using sutures, which are approved devices, for thispurpose. According to the council, although the procedure is legal, it can result in scars, infections and evenbrain abscesses. Another treatment for male-pattern baldness, hair implants made of high-density artificial fibers surgically implanted inthe scalp, was banned by the FDA in 1984 because it causes infection. This is the only device FDA has ever banned. PRODUCTS THAT DON'T WORK So-called "thinning hair supplements," "hair farming products" and "vasodilators" for the scalp will not promote hairgrowth, says Mike Mahoney, a spokesperson for the American Hair Loss Council. Thinning hair supplements are nothing more than hair conditioners that temporarily make your hair feel or look a littlethicker. The main ingredient in these products--polysorbate--is also found in many shampoos. Promotional materials forhair farming products claim that they will release hairs that are "trapped" in a bald scalp. Mahoney says these products, many of which are herbal preparations, can do no such thing. And so-called vasodilators do not increase the blood supplyto the scalp and do not promote hairgrowth. EVERYDAY HAZARDS While male- and female-pattern baldness results in permanent hair loss, other factors can cause temporary loss of hair. For instance, the drop in the level of estrogen at the end of pregnancy can cause a woman's hair to shed more readily. Two or three months after a woman stops taking birth control pills, she may experience the same effect, since birthcontrol pills produce hormone changes that mimic pregnancy. A major physical stress, such as surgery, or a majoremotional stress--positive or negative--can cause hair loss. "I've seen women start losing their hair before getting married," says Bihova. Even jet lag can have a similar effect. In most of these cases, the hormonal imbalance or stressful situation will correct itself, and the scalp will soon begingrowing hair again. But, says Bihova, since most women are extremely upset by even a temporary hair loss, manydermatologists treat these conditions with either topical steroid preparations or localized injections of low doses ofsteroids. Bihova emphasizes that these are local, not systemic, injections of steroids, therefore, the shots do not havethe same risk of dangerous side effects as systemic steroids. However, only a board-certified dermatologist shouldadminister this treatment, she says. The list of causes of temporary hair loss goes on: pressure on the scalp from wigs or hairdos that pull too tightly cancause it. A fever of 103 degrees Fahrenheit or more often causes hair loss six weeks to three months later. And somemedications can cause a temporary loss. These include vitamin A derivatives such as Accutane, cough medicines withiodides, anti-ulcer drugs, some antibiotics, beta blockers, antidepressants and amphetamines, anti-arthritis drugs, bloodthinners, some cholesterol-lowering agents, aspirin taken over long periods, some thyroid medications, and chemotherapy. YOU HAIR WHAT YOU EAT? Although nutrition does play a role in hair loss and in the overall health of your hair, only extreme nutritionaldeficiencies or excesses will cause hair loss. For instance, people with anorexia and bulimia may temporarily lose hair.So will others suffering from malnutrition. "It's pretty rare in the United States," says Bertolino. "If someone was on a real strange, restrictive diet, it couldhappen to them." Megadoses of some vitamins--particularly A and E--and an iron deficiency may lead to hair loss. People who claim they candetermine which vitamins are lacking in your diet by analyzing your hair, however, are not speaking from a scientificallysound basis. The test used with this type of hair analysis--atomic absorption spectrophotometry--is a legitimateanalytical chemistry method; however, used on hair, the results of this test do not correlate with nutritional status, says Shupack. "Because of the sociological importance of hair, a lot of people try to cash in on it," he says. "Hairanalysis is all witchcraft as far as I'm concerned." There are, however, a few legitimate hair tests for substances such as arsenic and lead. FOR BEAUTY'S SAKE Every time you shampoo, blow dry, perm, straighten or dye your hair, you damage it slightly, says Bertolino. For the mostpart, hair can withstand this type of treatment. But overzealous beautifying can damage the hair fiber, resulting in manybroken strands, and a frizzy, split-end look. For instance, if you bleach your hair and then have a bunch of perms donein a short time, you're heading for trouble. Misuse of hair cosmetics can cause the hair to break as it comes out of the scalp, says Frances Storrs, M.D., professor ofdermatology at the Oregon Health Sciences University. Permanent wave solutions break the bonds that hold hair togetherand then reform them. But with a perm that is not diluted right or not rinsed off properly, for instance, those bonds maynot re-form and the hair would soon fall out as a result. Fortunately, most professional hair dressers know how to useperms correctly, says Storrs. Most hair dyes are not as irritating as permanent solutions, mostly because they do not break the bonds between hairfibers and are therefore not likely to cause a hair loss, she says. However, a severe allergic reaction to hair dye couldcause hair loss. "The allergy is pretty common, actually," says Storrs. Permanent solutions can also cause allergicreactions, though that's a rare side effect. Other beauty-related manipulations of the hair can cause problems, too: hot irons, corn rows and braids may bring ontemporary or permanent hair loss. If the hair breaks often enough, the follicles may eventually not be able to producenormal hair, says Bihova. "If someone has a problem with thinning and excessive loss, we advise being gentle," she says."Don't use rollers; don't use blow dryers on a hot setting; don't wear tight hair styles." Rough shampooing may accelerateany loss, though it's usually not a problem in people with healthy hair. THE MEDICAL SIDE Some hair loss is the result of a type of immune disorder known as alopecia areata--some 2.5 million people suffer fromthis condition in which antibodies attack the hair follicle, causing the hair to fall out. Alopecia areata often causessmall, oval or circular areas of hair loss. However, in some forms of the condition, all the scalp hair falls out; inother forms, all body hair is lost. Although the loss is usually temporary, the condition can recur. Treatments includetopical steroids or the use of chemicals to produce an allergic reaction to start the hair growing again. Finally, chronic, systemic conditions--including one form of lupus, abnormal kidney and liver function, and hypothyroidismor hyperthyroidism--can affect the hair. If you're experiencing hair loss, see a doctor. He or she will want to order somebasic blood tests to rule out any medical cause of the condition. Consumer Beauty Products Division hair.Well-groomed, luxuriant, lustrous, hairHair Hair is a very important expression of who we are. It is often the first thing you notice when you meet someone, especially if they have nice hair.Well-groomed, luxuriant, lustrous, hair is, quite simply, beautiful and sexy. It can signify youth, health, strength and vitality. Many people don't feel right unless their hair looks perfect. If their hair looks good they feel great. Otherwise they say they are having a 'bad hair day'. This applies to men and well as women. Hair reacts to our state of well being. This includes our physical, spiritual, and emotional bodies. Emotionally, if we are in balance, our hair grows and looks beautiful as a personification of of our inner self. Under stress many people have hair loss or their hair looses its texture and sheen. One such disease is Alopecia Areata which is a highly unpredictable, autoimmune skin disease resulting in the loss of hair on the scalp and elsewhere on the body. Other factors that affect hair include: Physical illnesses - Diet - skin conditions - - hormones - shampoos & conditioners - hair dyes, perms - the water you use to wash you hair, as well as other factors that impact on our daily lives. The manner in which you care for your hair also various greatly. Some people just wash their hair and let it dry naturally, otherpeople spend hours blow drying and styling their hair. We all seem to find a style that is 'us' and keep it until something in our liveschanges - and we change our hair style along with it. The hair we are born with changes several times during our time on planet Earth. Some of these changes occur naturally, while others get a little help from hair stylists and barbers. The word Barber comes from the Latin word barba meaning beard. Itmay surprise you to know that the earliest records of barbers show thatthey were the foremost men of their tribe. They were the medicine menand the priests. But primitive man was very superstitious and the earlytribes believed that both good and bad spirits, which entered the bodythrough the hairs on the head, inhabited every individual. The badspirits could only be driven out of the individual by cutting the hair,so various fashions of hair cutting were practiced by the differenttribes and this made the barber the most important man in thecommunity. In fact, the barbers in these tribal days arranged allmarriages and baptized all children. They were the chief figures in thereligious ceremonies. During these ceremonies, the hair was allowed tohang loosely over the shoulders so that the evil spirits could comeout. After the dancing, the long hair was cut in the prevailing fashionby the barbers and combed back tightly so that the evil spirits couldnot get in or the good spirits get out. We visualize a young baby with short cute hair . . . as babies are usually adorable. Most babies loose their newborn hair, the newhair growing in with a different color and texture. Some babies remains bald until after a year old then grow in mountains of thickhair.. As children grow older boys seem to have fewer choices of style. The final style depends on the natural texture of the hair, theparents chose, and the society in which they live. But girls are a totally different story. This is where many factors are at play . . . Mothers who keep their daughters in short hair usually tell me it's because the child will not allow the mother to fuss with the child'shair or the child's hair is very thin and fine and looks bad when long. Other than that, many mothers like the long look which can styled differently and usually reflects that natural look. In the teens years the look is often long hair, which remains that way until after school when the person goes out into the world andsociety dictates the hair style that is best for them. In many cultures long hair is the norm for women - and sometimes men - in all age groups. How much time and energy you put into your hair varies with how important it is to you. If you are lucky you have 'wash and wear' hair that always looks great after it is washed and dries naturally. If you are like most people you have to create the illusion of how you see yourself by creating you hair color and style. Direction of hair growth on your head Ananova - January 2004 German scientists say whether the hair on your crown grows clockwise or anti-clockwise could affect the way your brain works. WORKING WITH OUR HAIR - Color: The right color accentuates one's aura - the person glows - Sometimes a few highlights can create the same illusion.- Many depressed people dye their hair black!- Perm or Straightening: It seems to be true that most curly-haired people like straight hair and vice versa.- Hair blowing: Used for style and held in place with hair spray- Curling irons- Gels of Mousse: Give the hair body and often shine- Extensions and Wigs: Can create any illusion that you desire - from the business look to the priestess - to the sophisticate. Manymen wear hairpieces that are undetectable. Wigs are also worn in different religions and after certain medical procedures - such aschemotherapy.- Styling: This varies with the hair texture you have and the nature of your lifestyle- Accessories placed in your hair: Jewels, fancy pins and other ornaments LONG HAIR - BACK TO NATURE Long hair is primal . . . It stirs and arouses something within us. . . . It is sexy. It references a return to the creational source - water - the flow of the collective unconscious. In many places woman style their hair in 'snail shell' curls turning to the right. This is linked with Fibonacci - The Spiral of Consciousness energy moving upward through the crown chakra. We are in the Age of Aquarius - The title song of the hit show 'Hair'. Aquarius represents the mother creator - the priestess and goddess the creational mythology - the return of feminine energies -the higher frequency energies/chakras. Aquarius is always portrayed with long flowing tresses - often flowing into creation. The Mother - Prime Creator - Isis - is always portrayed as tall, slim, with flowing long hair. She is sexy and alluring . . her hair naturally thick and often wavy. She is the Mother Nurturer - The Protector Mythological creational gods are portrayed with long hair - is a return to our natural state of being. LONG HAIRED MEN Jesus of Nazareth Zoroaster George Washington Thomas Jefferson Benjamin Franklin Yanni Willie Nelson George A. Custer Famous Native Americans (Red Cloud, Black Elk, Tecumpseh, Crazy Horse, Etc.) Silent Wolf Steven Seagal Beethoven Einstein Rasputin Michelangelo William Shakespeare Ascended Masters Creational Gods LONG HAIR IN FILM Women in movies - femme fatales - have almost always had long hair. They are soft, sensual, desirable. A woman letting loose her long tresses - that have been tied or pinned back, suddenly is seductive and alluring. Her energies are strong as she shakes looses her lion mane of hair. As we move back into the Aquarian - Feminine Age - we find our movies and TV shows filled with female heroines - who are tougherthan their male counterparts. These woman are always portrayed in the primal goddess look having long flowing tresses. Female heroines seem to be in vogue now. No longer are they quiet and demure waiting to be rescued. This is the age of Aquarius - the return of the feminine in strength and frequency. Country singers always have mountains of hair sometimes wearing wigs. Hair is all part of the illusion - the game. LOOKING YOUNGER Hair makes a statement at any age. I believe that longer hair, kept in a natural color, not too dark, is more youthful that short gray hair. For some reason when the hormones go . . . during menopause . . . many women cut their hair short and let it go gray. That's fine I guess if that is how you see yourself ...but I work with many people and unless gray hair is really flattering to you, itshould be replaced with a color that softens your aura and makes you look young. I have one client in her 60's who has had gray hair for 20 years. It is gorgeous, but she has very think wavy youthful hair and hasit style in a shoulder length hair-do that she blows out each day. Is it important to look and feel young? For most people this is the case especially in western societies where we are judge so much on appearance. When you look young - you send out the message - I am young and stronger and in control. When you look older - or sickly and poorly kept - as you age - it says, "I am depressed and not in control I am bored and gettinglaid back and perhaps lazy about who i am. I am boring. I have issues, etc. PSYCHOLOGICAL DISORDERS Many people who are mentally ill often tend to: - cut off random chunks of hair - dye their hair black - a true sign of depression - loss their hair in different parts of the head - do not properly maintain their hair - dye their hair based on a multiple personal disorder - twirl their hair - pull their hair out - called Trichotillomania Trichotillomania is a term coined by a French dermatologist in 1889to describe the compulsive or irresistible urge he saw in patients to pluckout their hair. The word trichotillomania is derived from the Greekthrix, hair, tillein, to pull, and mania, madness or frenzy. This nameis somewhat of a misnomer in that people with trichotillomania are not "mad","psychotic" or "crazy" as the name suggests. In psychiatry, trichotillomania, is classified as an Impulse Control Disorder. PHYSIOLOGY OF HAIR On the average there are over one hundred thousand strands of hair on a young adult. Blondes average about 140,000 strands, brunettes average 108,000 and redheads average 90,000. Hair grows at a rate of about 150mm a year and each individual hair lasts for two to six years before it falls out, the follicle has a rest for a while, while another takes its place. Hair grows quickest in young adult women aged between sixteen and twenty-four (who are often then at the height of their possible reproductive capacity) The quality and appearance of hair is influenced by overall health and diet as would be expected. Anorexics who starve themselves often have very fine, brittle hair deficient in various minerals. Hair conveys information about a person and their state of health-analysis of the hair can also tell what drugs they have taken. Long hair obviously suggests at least a recent history of good health. DREAM SYMBOLOGY To dream of hair in your dreams could suggest somestrong thoughts are floating around up there but a strong mentalpower can hold them in check. To dream of gray or silver hair could suggest wise thoughts. For a woman to dream that she has beautiful hair when in realityshe does not, denotes carelessness in her personality. For a man to dream of losing his hair, suggests too much generositywill put him in the poor house. To dream that you cut your hair close to your head may signifyregrets of a willful extravagance. ONCE UPON A TIME . . . Once upon a time . . .the story of human evolution was written. Everyone had long hair as there were no scissors. Once upon a time . . . in other magical and mythological places everyone had naturally long hair. . . LADY GODIVA She was the wife of Leofric, earl of Mercia; famous for her legendary ride through the city of Coventry. She was a benefactor ofseveral monasteries, especially that at Coventry, which she and her husband founded (1043). The legend about her, which firstappears in the chronicle of Roger of Wendover, states that her husband agreed to remit the heavy taxation on the people ofCoventry if she would ride naked through the town on a white horse. The story of Peeping Tom, the only person who looked throughthe closed shutters, did not enter the legend until the 17th cent. Michael Drayton (1613), Tennyson (1842), and others made LadyGodiva the subject of poems. A bronze statue of her by Sir William Reid Dick was erected in Coventry in 1949. SAMSON Because of certain resemblances some scholars have claimed that the biblical account of the career and exploits of Samson is buta Hebrew version of the pagan myth of Hercules. There is no serious proof to this. Still less acceptable is the opinion which sees inthe biblical narrative merely the development of a solar myth, and which rests on little more than the admitted but inconclusivederivation of the name Samson from shemesh - sun . Samson was supposedly the last and most famous of the Judges of Israel. The narrative of the life of Samson and his exploits iscontained in chapters xiii-xvi of the Book of Judges. After the deliverance effected by Jephte, the Israelites again fell into theirevil ways and were delivered over to the Philistines for forty years. An angel of the Lord in the form of a man appears to thebarren wife of Manue of the tribe of Dan and promises her that she shall bear a son who shall deliver Israel from the oppression ofthe Philistines. He prescribes abstinence on the part of both mother and son from all things intoxicating or unclean, and that no razor shalltouch the child's head, for he shall be a Nazarite of God. The angel bearing a similar message again appears to Manue as wellas to his wife, and it is only after his disappearance in the flame of a burnt offering that they recognize with great fear his celestialnature. The child is born according to the prediction and receives the name Samson, and the narrative informs us that the spirit of theLord was with him from his youth. Strangely enough this spirit impels him in spite of his parents' opposition to choose a wife from among the ungodly Philistines(Judges, xiv, 1-4). On a visit to Thamnatha, the town of his intended bride, Samson gives the first evidence of his superhumanstrength by slaying a lion without other weapon than his bare hands. Returning later he finds that a swarm of bees have taken up their abode in the carcass of the lion. He eats of the honey and theincident becomes the occasion of the famous riddle proposed by him to the thirty Philistine guests at the wedding festivities: Outof the eater came forth meat, and out of the strong came forth sweetness. In their inability to find the answer the guests, toward the end of seven days' feast, induce Samson's wife to coax him to reveal itto her, and not sooner has she succeeded than she declares it to her countrymen. Samson, however, in order to provide the thirtygarments pledged in the wager, goes down to Ascalon in the spirit of the Lord and slays thirty Philistines whose garments hegives to the guests who had declared the answer to the riddle. In anger he returns to his father's house, and his bride chooses one of his wedding companions for her husband. He returns later to claim her and is informed by her father that she has been given to one of his friends, but that he may haveinstead her younger and fairer sister. Samson declines the offer and catching three hundred foxes he couples them tail to tail, andhaving fastened torches between their tails turns them loose to set fire to the corn harvests of the Philistines which are thusdestroyed together with their vineyards and olive-yards. The Philistines retaliate by burning the faithless wife and her father, whereupon Samson makes a "great slaughter of them" and then retires to dwell in a cavern of Etam in the tribe of Juda. Three thousand Philistines follow him and take up their quarters at Lechi. The men of Juda, alarmed, blame Samson for this invasionand deliver him up bound to the enemy. But when he is brought to them the spirit of the Lord come upon him; he bursts his bondsand slays a thousand Philistines with the jawbone of an ass. Being thirsty after this exploit, he is revived by a spring of water which the Lord causes to flow from the jawbone. Later whileSamson is visiting a harlot in Gaza the Philistines gather about the city gate in order to seize him in the morning, but he, rising atmidnight, takes the gate, posts and all, and carries it to the top of a hill in the direction of Hebron. Subsequently he falls in love with a woman named Dalila of the valley of Sorec, who is bribed by the Philistines to betray him intotheir hands. After deceiving her three times as to the source of his strength, he finally yields to her entreaties and confesses thathis power is due to the fact that his head has never been shaved. The paramour treacherously causes his locks to be shorn and he falls helpless into the hands of the Philistines who put out hiseyes and cast him into prison. Later, after his hair has grown again he is brought forth on the occasion of the feast of the god Dagon to be exhibited for theamusement of the populace. The spectators, among whom are the princes of the Philistines, number more than three thousand, and they are congregated in,and upon, a great edifice which is mainly supported by two pillars. These are seized by the hero whose strength has returned; he pulls them down, causing the house to collapse, and perishes himselfin the ruins together with all the Philistines. RAPUNZEL Rapunzel was the most beautiful child under the sun. When she was twelve years old the Witch shut her up in a tower, in the middle of a great wood, and the tower had neither stairs nor doors, only high up at the very top a small window. When the old Witch wanted to get in she stood underneath and called out: "Rapunzel, Rapunzel, Let down your golden hair," Rapunzel had wonderful long hair which was as fine as spun gold. Whenever she heard the Witch's voice she unloosed her plaits, and let her hair fall down out of the window about twenty yardsbelow, and the old Witch climbed up by it. Many years past. Then one day that a Prince was riding through the wood and passed by the tower. As he drew near it heheard someone singing so sweetly that he stood still spell-bound, and listened. It was Rapunzel in her loneliness trying to while away the time by letting her sweet voice ring out into the wood. The Prince longed to see the owner of the voice, but he sought in vain for a door in the tower, but found none. One day, when he was standing thus behind a tree, he saw the old Witch approach and heard her call out: "Rapunzel, Rapunzel, Let down your golden hair." Then Rapunzel let down her plaits, and the Witch climbed up by them. So on the following day, at dusk, he went to the foot of the tower and cried: "Rapunzel, Rapunzel, Let down your golden hair," She let it down the Prince climbed up. At first Rapunzel was terribly frightened when a man came in, for she had never seen one before; but the Prince spoke to her so kindly, and told her at once that his heart had been so touched by her singing, that he felt he should know no peace of mind till he had seen her. Very soon Rapunzel forgot her fear, and when he asked her to marry him she consented at once. "For," she thought, "he is young and handsome, and I'll certainly be happier with him than with the old Witch." So she put her hand in his and said: "Yes, I will gladly go with you, only how am I to get down out of the tower? Every time you come to see me you must bring a skein of silk with you, and I will make a ladder of them, and when it is finished I will climb down by it, and you will take me away on your horse." They arranged that till the ladder was ready, he was to come to her every evening, because the old woman was with her during the day. The old Witch, of course, knew nothing of what was going on, till one day Rapunzel, not thinking of what she was about, turned to the Witch and said: "How is it, good mother, that you are so much harder to pull up thanthe young Prince? He is always with me in a moment." In her wrath she seized Rapunzel's beautiful hair, wound it round and round her left hand, and then grasping a pair of scissors in her right, snip snap, off it came, and the beautiful plaits lay on the ground. And, worse than this, she was so hard-hearted that she took Rapunzel to a lonely desert place, and there left her to live in loneliness and misery. On the evening of the day in which she had driven poor Rapunzel away, the Witch fastened the plaits on to a hook in the window, and when the Prince came and called out: "Rapunzel, Rapunzel, Let down your golden hair," she let them down, and the Prince climbed up as usual, but instead of his beloved Rapunzel he found the old Witch, who fixed her evil, glittering eyes on him, and cried mockingly: "Ah, ah! you thought to find your lady love, but the pretty bird has flown and its song is dumb; the cat caught it, and will scratch out your eyes too. Rapunzel is lost to you for ever -- you will never see her more." The Prince was beside himself with grief, and in his despair he jumped right down from the tower, and, though he escaped with his life, the thorns among which he fell pierced his eyes out. He wandered about for some years,as wretched and unhappy as he could well be, and at last he came to the desert place where Rapunzel was living. He heard a voice which seemed strangely familiar to him. He walked eagerly in the direction of the sound, and when he was quite close, Rapunzel recognized him and fell on his neck and wept. But two of her tears touched his eyes, and in a moment they became quite clear again, and he saw as well as he had ever done. Then he led her to his kingdom, where they were received and welcomed with great joy, and they lived happily ever after. Laser Hair Removal Since I began reporting on this topic in 1995, there has been considerable interest and considerable trial and error in the field of laser hair removal. Originally touted as a "miracle cure" by some, it was almost impossible for it to live up to its hype. In the following years, it's become evident that laser hair removal can be very beneficial for some consumers and completely worthless for others. What started as a small section of my site for TSs was getting so much mail that I opened up a second site for the general market. Most of the information now resides on hairfacts, but I will have TS-specific information available here. As I have been saying since 1995, I am not anti-laser. I am simply pro-results. I suspect some day they'll figure it out. The burden of proof is on laser companies. My position is as unbiased as possible: I simply require proof of permanent and safe results before recommending lasers. That is the only criterion I consider important Permanent hair removal is one of less than half a dozen things essential for most MTFs. Consumers or marketers who make unsubstantiated claims may be well-intentioned, but they do a disservice to our community by confusing the issue and causing people to make purchasing decisions based on false information. Adding to the confusion is that some lasers are allowed to claim they achieve permanent hair reduction. I discuss this at length on hairfacts. TS facial hair removal is extremely difficult to achieve without damaging surrounding skin, and some lasers and practitioners are better than others. It is vital that consumers understand as much as possible before committing to an expensive course of treatment. Using lasers to supplement electrolysis Some TS women have found this a helpful way to get dark facial hair under control. Anyone considering this should meet the following qualifications: Have a fair skin type and dark hair Be willing to lose time and money if it's not as effective as you hope it will be. If the goal is to get the face under control, it seems smarter to put money into a permanent solution rather than a possible stopgap measure. However, getting hit with a laser is probably smarter than going full-time before doing anything. Is laser a cost-effective way to clear a face prior to electrolysis? Laser treatment prices have been dropping precipitously as they are now being sold to anyone with the money to buy one. It's alomost certainly better than plucking in terms of damaging follicles and causing ingrowns (not to mention pain). My recommendations are always based on what I feel is the safest route, and I think lasers were rushed to market without adequate testing, leaving consumers paying to be guinea pigs. If This outlines the basic arguments. I personally recommend doing electrolysis exclusively, but for those considering supplementing electrolysis with laser, please read the section below. Discussion of pros and cons It is easier to kill a hair that is "recovering" from laser treatment. After a 2 week wait, I will work in the area that has been treated. Much less current is needed to kill these hairs because they are much smaller...therefore there will be less skin damage. he saves a lot of money and pain. You were right about my use of "fair" being a bit too narrow in defining skin type. The general recommendation with lasers is to use them only on the four fairest of the six Fitzpatrick skin types, and to avoid using them on most types of freckled skin. And the less contrast between hair and skin color, the more potential for problems. The other drawback to waxing or laser is that you delay completion by that much more with each use. If it takes 8 weeks for anagen hairs to return and be treatable, you've lost 8 weeks. If it keeps hair away six months, you lose half a year. Does it all balance out in total time saved? It very well could. I just like people to know as much as possible going in. Bethanne answered: I highlighted your comment about "8 weeks" because hairs in a treated area (by laser) are treatable with electrolysis in a much shorter time. The hairs are very small, but that is the best time to kill them. I think the time lapse is only 2 weeks...and the "kill rate" is so much higher that a client is foolish not to pursue this....not to mention that a microscopic hair that is destroyed will not damage the skin. Waxing for whatever reason does not "shrink" the hair nearly as much...I don't pretend to know why. I literally plucked every hair above my jawline for 18 months before I ever started electrolysis...I wanted to be beard-free that bad! Yet when I started electrolysis immediately after this these hairs were no smaller than the others. The EpiLight laser is much better than waxing. I also have freckled skin...I like how the laser lightens the freckles and the skin color is more uniform. J___'s advice: HAVE IT DONE ON A TEST AREA BEFORE YOU SPEND MONEY-- See if it works for YOU. Don't expect it to clear everything. Compare it to electrolysis dollar for dollar; I think for many people it is very cost effective. It does not replace electrolysis but should be used in conjunction with electrolysis. Expect some discomfort as the EMLA wears off. -- Report: permanent TS facial hair removal with laser [note: the following was posted to USENET by me in June 2001 following confirmation of a young TS woman who was able to remove all her facial hair with laser alone.] For about six years, I have been collecting consumer reports from TS women regarding effectiveness of various types of hair removal. Of particular interest has been laser hair removal, because it was rushed to market without adequate testing, and consumers are basically paying to be guinea pigs. Some women with light skin and black or brown hair have sent me anecdotal reports of satisfaction with various types of laser, while others were led to believe theyd have better results than they did. A few have claimed permanent hair removal, but when questioned further, they were actually supplementing laser with another method, or they hadnt completed treatment and werent qualified to make an assessment yet. A couple of people claiming laser permanence turned out to be repeating erroneous second-hand information or (even worse) were promoters for laser practitioners. Well, I am very pleased to announce that Ive just been able to confirm a report that meets my arbitrary benchmark for permanent hair removal using laser only. A TS woman has provided a report that she has gone over one year after final treatment without using any other method to remove or conceal facial hair. The basics are this: she was an ideal laser candidate with light skin and black hair. She also transitioned early (SRS at 22) and is now living stealth. She spent a total of $2000 on two longer ruby laser sessions and three shorter follow-ups, and she has now been hair-free for several years. She writes, "My skin is absolutely smooth like a genetic female now [ ] As far as hair removal now, I have none to remove. No bleaching, waxing, plucking... anything!" My report is not ideal, because I cant confirm certain information. The doctor who performed the procedure is no longer practicing, so I could not confirm information with him. My correspondent couldnt recall for certain which model of ruby laser she was using, although she knew it was made by Palomar. This woman is very stealth, so I cant divulge any personal information or provide contact information for others to confirm her report. However, we have spoken by telephone, and her story checked out to my satisfaction. Unfortunately, youll have to take my word for this, which is not the way I usually like to report things. What does this report mean for those considering what method of hair removal to use? As I have stated before, some TS women with light skin and dark hair have reported satisfactory results from a few laser treatment. Experts suggest that after 3 or 4 treatments, youve probably received about as much benefit as youll get. Its important to note that this womans report is the first complete success, even among other ideal candidates. Ruby lasers of the type she used have the most available clinical data and have been on the market the longest of the types allowed to claim permanent hair reduction. The picture from this data suggests that on average, consumers with relatively light skin and dark hair receiving three or four treatments have an average of about half to two-thirds of their hair reduced at 6 months or more. However, researchers note uneven outcomes and non-response rates of almost 30% in some studies. The reason some ideal candidates do not respond to treatment is not known. For details on published ruby laser clinical data, please see: http://www.hairfacts.com/medpubs/rubymed.html My correspondent had some electrolysis with unsatisfactory levels of regrowth and skin damage. She lives in one of the 16 states where the practice of electrolysis is not regulated, which means its even harder to find someone competent, especially competent enough to successfully remove androgen-induced facial hair. She wrote, "Electrolysis was a failure for me. Less than 20 hours left my face scarred and the hair returned. I wasn't sure if laser would be permanent, but I figured a lifetime of touchups was a far better deal than a permanently scarred face." This underscores the importance of finding competent practitioners. Facial hair removal is an investment, and everyone will see the results of that investment every day. I believe that scarring from electrolysis can sometimes hinder those whose desire is to assimilate into mainstream society. Bottom line She concludes, "I don't know how the other skin/hair colors fare, as I can only speak for myself. But in my experience it is very, very effective." Will it be this effective in all cases? No. Those with gray, blonde or red hair will probably see little or no permanent benefit from laser. Those with darker skin must be very aware about the risks of skin discoloration. Even those with light skin and dark hair need to weigh the risks of electrolysis and laser and decide what is the best option under their own circumstances. If you have light skin and dark hair, you might kick-start things with a few laser treatments, but only after determining if you can afford it if it doesnt work. Should you spend a few thousand on laser in hopes of saving a few thousand on electrolysis? Only you can decide. It could pay off, but then again it might not. Theres still not enough published data to say for sure what your chances are. However, its heartening to know that there is at least one case where laser has been a complete success, and I hope this report is the first of many! My final thoughts As I get more information from people who have first-hand experiences supplementing electrolysis with laser, I will include them. To summarize my current opinion: Almost anyone who uses laser will have to supplement it with electrolysisto achieve complete and permanent hair removal. There is no clinical data indicating treatment with laser will improve electrolysis efficacy. The only certain advantage is a temporary cosmetic advantage of no five o'clock shadow. However, since hypopigmentation or a gridlike pattern of regrowth might be the trade-off, I'd strongly recommend weighing the risks before opting for what seems like a quick fix. Is it worth a shot in your case? Only you can decide. I recommend reading my general market overview on laser hair removal top learn more about which lasers work and which ones don't. This outlines the basic arguments. I personally recommend doing electrolysis exclusively, but for those considering supplementing electrolysis with laser, please read my laser tips . Was this useful?
