In the past few years, medicine has made tremendous strides in the treatment of men's hair loss. With the advent of 5-alpha-reductace inhibitors such as Propecia and the evolution of surgical hair restoration, living with noticeable hair loss is no longer inevitable. For the first time in the history it is now possible to stop or slow the progression of hair loss and to replace lost hair through surgery with completely natural results. However, with that said, the vast majority of hair loss treatments being marketed today are still nothing but "snake oils." You may have seen the ads in the back of men's magazines, you've heard the commercials on the radio, and you've seen the infomercials promoting miracle treatments for hair loss. The bottom line is that most advertised "treatments" do not work for the prevention and treatment of hair loss. If a hair loss treatment is not approved by the FDA or recommended by the American Hair Loss Association, chances are you are wasting your time and money. Remember that successful treatment of hair loss is greatly dependent on early intervention. It is critical to begin treatment with an effective product as soon as you notice the onset of hair loss. The following two treatments have been clinically proven to successfully treat hair loss in men to varying degrees. Finasteride (Proscar, Propecia) Finasteride is the generic name for the brand name drugs Proscar and Propecia. Finasteride was originally developed by the pharmaceutical company Merck as a drug (Proscar) to treat enlarged prostate glands. During the trials on men with prostate problems, researchers noted an intriguing side effect: hair growth. Since finasteride had already been approved by the FDA to treat enlarged prostates in men, Merck decided to pursue the possibility of developing finasteride as the first pill to treat male pattern baldness. Minoxidil, a topical liquid solution, was already on the market (see below). In December 1997, the FDA approved a 1mg dose of finasteride for the treatment of androgenetic alopecia (male pattern baldness) in men. Propecia is the first drug in history to effectively treat male pattern baldness in the majority of men who use it. How Finasteride Works Finasteride's hair-raising success is due to its ability to specifically inhibit 5-alpha-reductase, the enzyme that converts testosterone into a more potent androgen dihydrotestosterone (DHT). Propecia's 1 mg dose of finasteride can effectively lower DHT levels in the scalp by as much as 60% when taken daily. It is DHT that shrinks or miniaturizes the hair follicle, which eventually leads to baldness. This 60% reduction in DHT has proven to stop the progression of hair loss in 86% of men taking the drug during clinical trials. 65% of trial participants had what was considered a substantial increase of hair growth. At this point, the only truly effective medically proven way to arrest the hair loss process is to lower DHT levels. The American Hair Loss Association recommends finasteride as the first line of attack for all men interested in treating their male pattern baldness. Minoxidil (Rogaine) Minoxidil was the first drug approved by the FDA for the treatment of male pattern baldness. For many years, minoxidil, in pill form (brand name Loniten), was widely used to treat high blood pressure. Just like finasteride, researchers discovered a very interesting side effect of the drug. People taking the medication were growing hair in unexpected places, such as on their cheeks and the back of their hands. Some people grew hair on their foreheads. Some enterprising researchers had the notion that applying minoxidil topically, directly on the head, might grow hair on balding areas. It did, to varying degrees depending on the extent of the hair loss, but at the time it was revolutionary. While minoxidil has been clinically proven to slow the progression of hair loss and regrow some hair, most experts see it as a relatively marginally effective drug in the fight against hair loss. Since minoxidil has no effect on the hormonal process of hair loss, its positive effects are at best temporary and usually yield somewhat disappointing results. The American Hair Loss Association still recommends the drug for those who have not responded favorably to finasteride treatment or for those who would like to add another product to their regimen. The AHLA does not recommend minoxidil as the first line of attack for men suffering with male pattern baldness, but does recognize it as an effective treatment for a small percentage of its users. ©2005 American Hair Loss Association. How do you reckon you rate in the looks department? Could you look better? Or for you, is it impossible to improve on perfection? Is a vain person really vain or someone who thinks they're better-looking than you or me? Not that hard, I'd have thought. If these questions have crossed your mind - I bet they have - you're watching the right program. That's what we're on about tonight - the whole "Mirror, mirror on the wall" thing. And what better place to start than literally at the top? DR NICK CARR: Like death and taxes, for some of us, baldness seems inevitable. And it's not just the blokes, as half of all women eventually become thin on top. But with modern treatments, hair loss is no longer the recession we have to have. (To man inspecting head) So tell me the awful truth, Rod. DR ROD SINCLAIR, DERMATOLOGIST: Well, you definitely are going bald. DR NICK CARR: If the news you're losing it on top worries you enough to want to redress it, your basic options are cover-ups, surgery, and some types of medication. DR ROD SINCLAIR: For men, there's one tablet that works, which is finasteride - Propecia. And there's one lotion that works, minoxidil, or Rogaine is the marketing name. DR NICK CARR: Finasteride is a hormonal treatment that affects two different parts of the body. One is the hair follicle, which is what we want, and the other can be the prostate gland, so you might want to check that out with your doctor. And half of 1% of men report some decrease in their libido. The other side effect is cost, which is about $60-$80 a month. Now, the alternative medication is minoxidil, which is about $50 a month. This started life as a blood pressure medication but they found that it had the side effect of making people grow hair all over the place. So they turned it into a lotion which you can buy off the shelf and you just rub it in. DR ROD SINCLAIR: It works for about a third of men, who will get additional hair regrowth. It also slows down the rate of progression to some degree in another third, but for about a third of men, no effect at all. PHOTOGRAPHIC IMAGES OF BALDING CROWNS ON WOMEN DR NICK CARR: There are two different tablets suitable for women, and they can also use the minoxidil lotion. DR ROD SINCLAIR: We've found we can arrest further progression of hair loss in 90% of women and we can stimulate regrowth in about 30%. The time to seek treatment is at the early stage, because they're much better at preventing further progression than they are at stimulating regrowth. DR NICK CARR: If the pharmaceuticals don't appeal or don't work, you might want to try a more traditional approach. (Puts on wig) Rod, tell me about hair transplants. You know, if you've gone completely bald, is that an option? DR ROD SINCLAIR: Well, if you do your research and find a good surgeon, then the results you can achieve with hair transplantation are very natural, and certainly much better than what was achievable even 10 years ago. DR BARRY WHITE, HAIR TRANSPLANT SURGEON: It's very simple, Nick. Take a strip of skin from the back, close it up, give it to the technicians. They divide it into units of ones, twos and threes. Then they place it in the scalp. It's as simple as that. DR NICK CARR: So you're putting each individual hair in place, one at a time? DR BARRY WHITE: Correct, yes. 3% of our current clients are women, and it works perfectly for them, because they retain their front line, they have a thinning area at the top here - you just fill that up, it works beautifully for them. DR NICK CARR: Now, what's the cost of this sort of procedure? DR BARRY WHITE: Look, it depends on how many grafts we do. But if a guy wants the front done, up here, you're looking at about $11,000 - $11,500. If he wants the back done - we can't do them at the same time - we generally do that for about $9,000. So you're looking at about $20,000 all up. DR NICK CARR: How long is this sort of hair graft going to last? DR BARRY WHITE: It'll last a lifetime. In fact, you'll take it to the grave. DR NICK CARR: (To patient undergoing transplant) So, Frank, what made you decide to have this done? FRANK FALVO: I looked at the options a few years ago, with the plugs, and I found they were unsatisfactory. I didn't like the results. I went to my doctor and I tried Rogaine and some of the other lotions, minoxidil. I was too far gone for that. So my brother suggested to come to Barry. DR NICK CARR: How much is this costing you? FRANK FALVO: Well, it isn't cheap. After all the net effects, it comes to about $11,500, after it's all netted off with the government, so... DR NICK CARR: Hey, but $11,000 - you could have spent that on a wig. FRANK FALVO: I had to sell my Telstra shares. They've just gone up, so I'm really spewing. DR NICK CARR: Barry, at present we're taking hair from the back of someone's head, but is there any way of making hair - grow it in the lab, for instance? DR BARRY WHITE: Like skin culture or hair culture? They've been working on that for about five years and I expect that 5, 10 years down the track, it will be available. It's not available at the moment. DR NICK CARR: It might be ready when I need it? DR BARRY WHITE: Certainly. I mean, you need it now. DR NICK CARR: (Laughs) GEORGE NEGUS: That's why I love this program. How real was that?! But $20,000 for a full transplant? I think I'll just stick with the rug.
