Hair Loss & Treatment Center Patient Education Lecture Series Schedule air loss, or alopecia (pronounced ah-low-PEE-sha) affects millions of people: men, women of all ages and all ethnic groups. The cause of alopecia is complex and includes hormones, genetics, medications, stress, severe illness, malnourishment, infections, and autoimmune diseases. The Department of Dermatology of the College of Physicians & Surgeons at Columbia University recognizes the serious impact of alopecia on people's lives and has assembled a team of world-renowned basic scientists and highly experienced clinicians to create the Hair Research & Treatment Center. Here, patients have access to the most recent scientific developments, diagnostic techniques and treatment options. They will be evaluated by compassionate, dedicated dermatologists specializing in hair disorders. This innovative, comprehensive treatment center will bring the specialty of hair loss treatment and research into the 21st century. New Discovery ! Benefit From Genetic Breakthrough Patients of the Hair Research & Treatment Center will also benefit from the recent exciting discovery by Angela Christiano, Ph.D. and her research team in the Department of Dermatology the first human gene associated with hair loss! The new gene, called hairless, is linked to a severe form of inherited baldness and may be the trigger that turns on the entire human hair cycle. Eventually, understanding the genes that control the hair cycle may lead to more effective treatments for various forms of hair loss. The research suggests that the gene initiates a cascade of events that stimulate hair growth. Each step along this pathway may provide new clues for other forms of alopecia. We have begun a new search for genes involved in Androgenetic Alopecia and Alopecia Areata. To do this, Dr. Christiano will begin collecting pairs of brothers and pairs of sisters who began losing their hair about the same time. Needed are 200 pairs of patients (400 individuals) within each of the following groups: Pairs of brothers with Androgenetic Alopecia Pairs of sisters with Androgenetic Alopecia Pairs of brothers with Alopecia Areata Pairs of sisters with Alopecia Areata If you would be interested in participating in studies aimed at mapping additional genes for alopecia, researchers would be interested in learning more about your family. To find out how The Research & Treatment Center can help you, or for a referral to one of our physicians, please contact Catherine Donovan-Stevens at (212) 305-5317. Hair Loss Can Be Treated Treatments are available for different forms of hair loss. The Hair Research & Treatment Center offers patients a comprehensive evaluation, including: physical examination history laboratory tests (as needed) scalp biopsy (as needed) microscopic evaluation of hair (as needed) After a complete assessment, appropriate medical and surgical treatments, as well as cosmetic options are discussed with the patient. Medical treatment may include FDA approved medications or new agents that are being studied in our Clinical Research Unit. Types Of Hair Loss The types of hair loss that can be treated at the Hair Research & Treatment Center include: Androgenetic Alopecia Male or female pattern baldness is due to a combination of genetics and hormonal influences on the hair. In men, this usually consists of a focal area of hair loss on the top of the head and at the temples which gradually increases. Women tend to have a more diffuse pattern of hair loss. Alopecia Areata Believed to be caused by an autoimmune reaction. Manifests itself in three different ways: Areata, Totalis and Universalis. Alopecia Areata is focal and patients can have one or more patches which may or may not spontaneously resolve. Alopecia Totalis is total loss of hair on the scalp, and Alopecia Universalis is complete loss of all hair on the scalp and body. Treatment may be as simple as changing a current medication, treating an infection, correcting hormonal imbalances, or using medication to halt the balding process. Other approaches relate to surgical treatments to improve appearance. Surgical Procedures & Techniques Today, several surgical procedures are available to treat hair loss: Hair Transplantation Involves grafting, a process where strips of hair bearing skin are removed from the back of the head and are dissected into smaller sections and implanted in the balding portion of the scalp. Scalp Reduction Involves the surgical removal of a portion of the balding area, thereby in effect shrinking the bald spot. Scalp reduction is used to reduce the front to top balding syndrome associated with male pattern baldness. Flap Surgery Involves the transfer of a large portion of hair and scalp from the area behind the temples to the forward, balding part of the head, creating a new hairline. Additional surgical techniques are also available in some circumstances. Your dermatologic surgeon can discuss these procedures with you. Importance of Clinical Trials As a part of Columbia University's College of Physicians and Surgeons' Department of Dermatology, the Hair Research & Treatment Center is able to offer its patients the benefits of the wide variety of clinical trials, many in conjunction with major pharmaceutical companies. A clinical trial is a research study conducted by physicians and other medical professionals to evaluate new treatments or procedures. Studies are designed to answer specific questions and to find new, innovative ways to help patients. Patients can benefit from clinical trials because the newest therapies are available to them before they are marketed to the general public. he following is a listing of the doctors affiliated with the Hair Research & Treatment Center through Columbia University College of Physicians and Surgeons, Department of Dermatology: Faculty David R. Drugs for hair loss Drugs for hair loss Drugs for hair loss... Do the "wonder drugs" work for restoring hair growth for pattern baldness? Here is a list of the most popular, widely used products, and the results of some studies about the products. 1. Minoxodil(Approved by the FDA) Most commonly referred to as Rogaine. Although it has been around since 1988 it was only approved for over-the-counter sales in 1996. About 25 percent of the men report some hair regrowth and about 20 percent of the women. There is a stronger version out for men, but it is unsafe for women to use. The extra-strength Minoxodil may work about 45 percent better. The cost is about $300.00 a year, and the hair regrowth stops when the treatment is discontinued. This product seems to be most effective in early stages of hereditary baldness. More than regrowing hair Minoxodil seems to help people retain the hair that they do have with more success. Comes in a cream form that you massage into the scalp. 2. Finasteride (Approved in 1997) is marketed as Propecia. It was the first approved treatment for pattern baldness that came in a pill form. The dosage is one pill a day and it may take several months before you can see any positive results. It is recommended that if you haven’t seen any new hair growth in 12 months that you discontinue treatment. Clinical trial results indicate that more than 80 percent of men taking finasteride may experience a slowing of hair loss, and more than 60 percent may show some new hair growth. The cost is about $50.00 a month. This drug is not safe for women to take and it poses very real danger for women of childbearing age. Women should not touch or handle the tablets because the absorption of the drug can cause birth defects in male fetuses. It has been shown to be safe for women when their partner is taking finasteride. Questions and Answers Q. What aren’t these treatments indicated for? A. Nonhereditary baldness Hair loss due to illness or childbirth Children and adolescents Q. What is really new and promising? A. (1.) Revivogen - This is a topical solution that can be used by both men and women. It contains every growth stimulant and DHT inhibitor currently known to science, in one solution; (2) the substance Folligen, a non-drug product meant to improve scalp and follicle health and vitality; (3)the new product Tricomin which uses Copper Peptides to stimulate naturally dormant follicles into their growth phases while it inhibits the damaging work of DHT. Tricomin can be used in conjunction with Rogaine and Propecia, providing additional benefits to the follicle and surrounding environment;(4) Proxiphen/Proxiphen-N is a topical formulation of many of the nonprescription hair loss treatment agents, similar to the product mentioned above. However since it also contains prescriptive agents it needs to be prescribed by your physician after a diagnosis of balding. As with most things in life, your best line of attack is education. The Internet is full of those newsgroups, message boars, chat rooms and product pages where you can find out what real people say about the products. There are Internet sites devoted to discovering products that are total "quackery." TheSite helps you get rid of any unwanted hair safely. Shaving Cheap and easy, but needs to be done every couple of days. Do this in the bath or shower using a foam or shower gel and a good razor. (Note if your razor is clogged beyond repair, or rusting, it needs binning or you'll end up with cuts and infections.) Shave in smooth down-up movements (i.e. ankle-knee, breast-head). Moisturise legs afterwards. Suitable for: Legs, underarms. Waxing Oooouucccchhh. And again Ouuuuuuccccccccccccchhh! Now if that hasn't put you off, you must have heard of the promises of living three weeks fuzz-free. You may want to go to the salon rather than do it at home, so you don't bottle it halfway through. Waxing can leave redness, so get it done a couple of days before that hot date. If you do wax at home, read the instructions carefully first. Suitable for: Legs, bikini line, eyebrows (if done professionally). Depilatory creams The major downside of creams is that they smell nasty, and this smell lingers both on you, your towels and your bathroom. They can also burn if you get them in the wrong places, they're prone to patchiness, and they need to be used every three days or so. Normally you apply cream thickly and evenly over the area, wait around five minutes, check a small area to see if the hair comes off, and if not wait a while longer (but no more than 10 minutes) before removing it all with a damp flannel. Read the pack instructions to be certain, and do a test patch first in case of a reaction. Suitable for: Legs, bikini line, underarms. Sugaring This technique is supposed to be less painful than waxing, plus you can wash and reuse the cotton strips. Test a small area first in case of allergy. You warm the sugaring mixture by placing in hot water, when it becomes spreadable, apply it in the direction of your hair growth. Then cover with cotton strips and pull the strips in the opposite direction of hair growth. Results last a few weeks. Suitable for: legs, bikini line, eyebrows (professionally). Bleaching Not a removal technique as such, more an effective disguise, bleaching lasts between 2 and 4 weeks. It involves lightening the hair on your face/ arms in much the same way as you would go bottle blond. Salon prices are normally only a few quid, so go see the professionals. Suitable for: Arms, facial hair. Electrolysis A thin needle is inserted into the hair follicle before sending an electrical charge down to the base of the follicle, causing the hair to fall out. As each hair is treated individually this takes a few trips to the salon before they are all done, and leaves some temporary redness. Results are (almost) permanent. Suitable for: Facial hair. Laser hair removal This new treatment deactivates the hair follicles by laser causing each hair to fall out. After a few treatments whole areas of the body can be left permanently hair-free, apparently. Works best on dark hair. Suitable for: Most places including bikini line, back, arms, legs, underarms, face.
